Message frequency varies based on your care needs, appointments, and service participation. You may receive:

      • Follow-up messages

      • Preventive care reminders

      • Clinic updates

      • Health-education messages

      • Program outreach messages

    Message and data rates may apply depending on your mobile plan.

    3. User Responsibilities

    You agree that:

        • The mobile phone number you provide is your own or you have permission to use it.

        • You will notify us if your phone number changes or is reassigned.

        • You will not use our messaging system for unauthorized or unlawful purposes.

      4. Opting Out

      To stop receiving texts, reply “STOP” at any time.
      To request help, reply “HELP.”

      Stopping SMS reminders will not affect your care or your ability to receive medical services from Elevate.

            • Damage caused by viruses, network errors, or unauthorized access
