Elevate Health Center provides health care services to residents of Sun Valley and surrounding communities to ensure that all patients have access to the care they need regardless of their ability to pay. Our mission is to elevate the well-being of the patients we serve.

Elevate Health Center’s Sliding Fee Discount Program provides discounts for essential health care services for uninsured and underinsured patients. Discounts are offered based on family size and income. Please contact our office to learn more about how you can apply for the Sliding Fee Discount Program.
